The answers are as required by the CNESST and Montreal Sprinklers inc .. We ask you to answer this survey in order to assess that your presence is safe at the project and to submit this form to the head office of Montreal Sprinklers inc . as well as the Health and Safety (CNESST) manager, or the superintendent if no CNESST manager is assigned to the site.
All employees must complete this form every day for each of the places visited.
